Direct Access / No Office Appointment Colonoscopy 
Most patients can be scheduled for a colonoscopy without an office visit.   Please complete the following three forms..The forms can be completed electronically or you can print the pdf form.  Completed paperwork can be emailed to [email protected], faxed to 860-606-0203 or mailed to our office.  You can also call our office and we will mail you the forms.  

If you have not been vaccinated for Covid-19, you will need to have a Covid-19 test 3 days before your procedure. 
If you are fully vaccinated you do not need a Covid-19 test before your procedure.  We ask that you provide a copy of your vaccination card

Locations for Procedures

Trinity Health of New England - 114 Woodland Street, Hartford, CT - The parking garage can be accessed from Woodland Street or Collins Street.  Proceed to the
     Admitting Department on the second floor.  You will then go across the hall to the Endoscopy Unit.
St. Francis GI Endoscopy Center - 360 Bloomfield Avenue, Suite 204, Windsor, CT.

Insurance

Patient Responsibilities:
- Please call your insurance company prior to your procedure.  It is important to contact your insurance company to verify your covered benefits and deductibles.
- Obtain any necessary referrals from your primary care provider.
- Notify our office immediately if your insurance changes.

Our office will obtain any required precertifications.  A precertification is not a guarantee of payment.

You may receive separate bills from our physicians, the facility where you have your procedure, the anesthesiologist and the pathologist and/or laboratory.

Medicare Patients:  Medicare covers a screening colonoscopy once every 10 years, but not within 4 years of a flexible sigmoidoscopy.  Please call your Medicare provider to verify coverage.

Our physicians can not change, add or delete a diagnosis so your insurance will cover your procedure(s).

Cancellation Policy
Please contact as soon as possible if you need to cancel or reschedule your procedure.

You may be charged a $150.00 cancellation fee if you cancel less than 72 hours prior to your procedure.  If you cancel or reschedule your procedure 3 times or more you may be charged a $50.00 fee.
We do not cancel procedures due to inclement weather.  St. Francis GI Endoscopy Center will close if the Governor/State of Connecticut closes the roads.

Medications

You may stay on all of your regular medications, vitamins and/or herbal supplements except Diabetes Medication, Iron and Blood Thinning Medications.  You do not need to stop Aspirin

Blood Thinning Medications:
Stop Eliquis, Xarelto, Pradaxa and Brillinta.  You will be instructed how long to stop these medications by your Gastroenterologist or the provider who manages these
     medications.
Stop Plavix (clopidogrel) 7 days prior to your procedure unless otherwise instructed by your Gastroenterologist or the provider who manages this medication.
Stop Coumadin (warfarin) 4 days prior to your procedure unless otherwise instructed by your Gastroenterologist or the provider who manages this medication.

Diabetes Medications: Do not take your diabetes medication on the morning of your procedure unless instructed by your Gastroenterologist or the provider who
     manages these medications.

Iron:  Stop Iron pills 5 days prior to your procedure.

Do not take the following medications on the morning of your procedure:
     ACE Inhibitors:  Lotensin (benazepril), Vasotec (enalapril), Prinivil/Zestril (lisinopril), Univasc (moexipril), Aceon (perindopril). Accupril (quinapril), Altace (ramipril),
          Mavik (trandolapril), captopril, fosinopril
     Angiotensin II Receptor Antagonists:  Edarbi (azilsartin), Atacand (candesartan), Teveten (eprosartan), Avapro (irbesartan), Cozaar (losartan), Benicar (olmesartan),
          Micardis (telmisartan), Diovan (valsartan)
