Direct Access / No Office Appointment Colonoscopy 

Most patients can be scheduled for a screening colonoscopy without an office visit.   Please complete the following 3 forms.  They can be completed electronically or you can print the pdf form.  Completed paperwork can be emailed to [email protected], faxed to 860-606-0203 or mailed to our office.  You can also call our office and we will mail you the forms.  .

Locations for Procedures

Trinity Health of New England - 114 Woodland Street, Hartford, CT - The parking garage can be accessed from Woodland Street or Collins Street.  Proceed to the
     Admitting Department on the second floor.  You will then go across the hall to the Endoscopy Unit.
St. Francis GI Endoscopy Center - 360 Bloomfield Avenue, Suite 204, Windsor, CT.

If you have not been vaccinated for Covid-19, you will need to have a Covid-19 test 3 days before your procedure.

If you are fully vaccinated you do not need a Covid-19 test before your procedure.  We ask that you provide a copy of your vaccination card
